Job Description
Sentara Rockingham Memorial Hospital is hiring a Community Health worker. The Community Health worker will work in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care. The role requires a High School Diploma or Equivalent and a valid driver's license. Experience working in a community-based setting and knowledge of medical terminology are preferred.
Requirements
- Collaborate with the Case Manager and other healthcare team members
- Provide comprehensive and coordinated care
- Engage in interventions to improve self-management skills and quality of life for patients with chronic and/or complex illness
- Provide education and behavior modification strategies
- Provide social support to patients and families
- Act as a patient advocate and healthcare navigator
