Job Description
Medical Social Worker responsible for provision of medical social work services to patients in a hospital setting.
Requirements
- Master’s Degree – Social Work
- Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI within 90 days of hire
- Certified Case Manager (CCM) - Certification - Commission For Case Manager Certification (CCMC) within 1 year of hire
- 1 year of case management experience
- Experience in hospital setting (Preferred)
- Experience with discharge planning (Preferred)
