Job Description
Sentara is hiring a Clinical Pharmacist at our Proprium Specialty Pharmacy in Chesapeake, VA. The role provides telephonic comprehensive medication management services to patients within Sentara's Specialty Pharmacies, ensuring safe, effective, and evidence-based use of specialty medications through patient counseling, therapy monitoring, and coordination with providers.
Requirements
- Bachelor's Level Degree in Pharmacy
- Virginia Pharmacist License
- Basic Life Support (BLS)
- Minimum of 2 years of experience in Clinical or Ambulatory Pharmacy
- Experience with MTM services
- Collaborative care coordination, chronic complex disease patient counseling, and clinical assessment experience
- Therigy EMR experience
