Job Description
Sentara is hiring a Clinical Pharmacist at our Proprium Specialty Pharmacy in Chesapeake, VA. The Clinical Pharmacist serves as a key member of the Ambulatory Clinical Support team, providing telephonic comprehensive medication management services to patients within Sentara’s Specialty Pharmacies.
Requirements
- Bachelor's Level Degree in Pharmacy (Required)
- Doctorate Level Degree in Pharmacy (Preferred)
- Virginia Pharmacist License (Required)
- Basic Life Support (BLS) (Required)
- Minimum of 2 years experience in Clinical or Ambulatory Pharmacy (Preferred)
- Experience with MTM services (Preferred)
- Collaborative care coordination, chronic complex disease patient counseling, and clinical assessment experience (Preferred)
- Therigy EMR experience (Preferred)
