The role involves managing client needs, building relationships, and collaborating with a multidisciplinary team.

Requirements

• Provide individual intensive case management and outreach.
• Develop treatment/service plans.
• Develop and maintain patient records.
• Collaborate with other patients and SRC teams.
• Perform adult initial assessment, progress notes, and discharge.
• Meet productivity goals and provide clinical information.
