Social Worker - Temp to Hire

We are seeking a Mental Health Clinician/MSW/Social Worker to provide high-quality therapeutic services and outreach to patients in a team-based environment. This role emphasizes a balance of love, care, and compassion, as part of a larger system focused on improving outcomes for those affected by homelessness and mental health challenges.

Requirements

• Provide individual and group counseling services.
• Evaluate patient needs and develop treatment/service plans.
• Build relationships with patients.
• Maintain accurate patient records.
• Ensure alignment with SRC model standards.
• Collaborate with SRC team.
• Perform Adult Initial Assessment, Discharges and Progress Notes.
• Meet productivity goals.
• Assist in providing clinical information.
• Adhere to all COVID policies.
