Job Description
ServiceNet is seeking a compassionate and resourceful Case Manager to support individuals experiencing homelessness at their Individual Shelter in Pittsfield, MA. The role involves conducting intakes, providing case management, and connecting guests with vital community services to aid their stability and transition to permanent housing. Requirements
- Conduct intakes and assessments for guests.
- Provide ongoing case management throughout the stay and during transition to stable housing.
- Engage individuals in the local community.
- High school diploma or equivalent with additional training.
- Valid driver’s license, personal vehicle, and acceptable motor vehicle record.
- At least one year of experience working with individuals experiencing low income, mental health conditions, and/or addiction.
