Requirements
- Identify, resolve, and prevent medication-related problems to ensure safe and effective pharmacotherapy.
- Conduct comprehensive assessments for chronic disease management under collaborative practice agreements or clinic protocols.
- Stratify patients based on clinical risk to prioritize interventions and tailor care plans accordingly.
- Provide individualized counseling on medication use, lifestyle modifications, and disease state education.
- Address barriers to adherence, including cost, access, and health literacy, through targeted interventions.
- Monitor and evaluate medication regimens to ensure clinical criteria are met, including therapeutic goals, adverse effects, and drug interactions.
- Work on-site in close coordination with interdisciplinary teams, including both in-person and hybrid care models.
- Ensure all clinical criteria are in clinical compliance with guidance from FDA, clinical guidelines, and appropriate evidence-based literature.
