Therapy Care Pharmacist

Soleo Health is seeking a Therapy Care Pharmacist to support our Specialty Infusion Pharmacy in Centennial, CO. The ideal candidate will have prior experience in home infusion and/or specialty pharmacy, a Doctor of Pharmacy (Pharm D) or R.Ph with relevant clinical experience, and applicable active state licensure. The Therapy Care Pharmacist will utilize national and local resources to facilitate high quality care of patients with specialty pharmacy needs, collect and assess clinical data, implement and monitor care plans, communicate with prescribers, and dispense prescription medications and supplies.

Requirements

• Prior experience in home infusion and/or specialty pharmacy preferred
• Doctor of Pharmacy (Pharm D) or R.Ph with relevant clinical experience
• Applicable active state licensure
• Post graduate residency and/or training
• Prior experience with CPR+ pharmacy computer system preferred
