Job Description
Southwest Care Center is seeking an experienced Population Health Manager to support population health and clinical quality improvement activities for its locations in Santa Fe and Albuquerque, NM. The ideal candidate will have a minimum of three years of experience working as an LPN/RN, preferably with clinical quality roles, and a current unrestricted NM nursing license.
Requirements
- Minimum three years working as an LPN/RN
- Associate degree in nursing with bachelor's degree preferred
- Current unrestricted NM nursing license
