Hospice Transition Nurse

St. Croix Hospice is seeking a Hospice Transition Nurse to guide patients and families through the end-of-life journey. The nurse will work with hospitals, physicians, and nursing homes to provide education and increase referral sources.

Requirements

• Current Registered Nurse (RN) license in state(s) of practice
• Ability to pass DHS background study
• Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record
• Graduate of an accredited school of nursing
• 1+ years of recent experience in hospice/home care preferred
• 2+ years' experience in Nursing, preferred
