Hospice Transition Nurse

Work as a Hospice Transition Nurse at St. Croix Hospice, guiding patients and families through the end-of-life journey. Provide education to referral sources and facilitate transition of care for patients. Mentor new RNs and establish relationships with hospital staff.

Requirements

• Current Registered Nurse (RN) license in state(s) of practice
• Ability to pass DHS background study
• Valid State Driver's License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record
• 1+ years of recent experience in hospice/home care preferred
• 2+ years' experience in Nursing, preferred
