Case Manager I

St. Joseph Center is seeking a Case Manager I to work with clients who meet criteria for chronic homelessness and severe mental health disorders. The Case Manager will provide intensive case management services, linkage services, and support clients in applying for subsidized housing and accessing community resources. The position requires a highly motivated individual with excellent organizational skills and ability to work in a fast-paced environment.

Requirements

• Develop working relationships with other non-profit agencies
• Present findings to collaborative agencies
• Collaborate with interdisciplinary team members
• Assist with developing and implementing client treatment plans
• Conduct ongoing case management assessments
• Identify and utilize client strengths
• Provide ongoing intensive case management services
• Ensure smooth transition into permanent housing
• Provide crisis management
• Communicate with landlords and other collaborating agencies
• Participate in meetings with collaborative agencies and other community meetings
