Case Manager I - Youth Resource Team

St. Joseph Center is a nonprofit community organization that assists people without regard for religious affiliation or lack thereof through comprehensive case management and integrated social service programs. The Case Manager works as a team member with several social service professional and paraprofessional staff ensuring the delivery of quality care as well as adhering to the mission of St. Joseph Center.

Requirements

• Provide case management to assist vulnerable youth to re-engage in the educational system, attain economic viability and achieve personal resiliency.
• Provide targeted outreach, engagement and education activities to Santa Monica youth that meet the criteria as “Opportunity Youth” services in order to increase education, economic viability, and personal resilience.
• Help prospective youth and their families access needed to community resources and supports.
• Assist in establishing eligibility for services as defined by agency and funding sources.
• Collaborate with partner agencies to ensure integrated services.
• Participate in regular Santa Monica Youth Resource Team meetings as well as agency and/or funders.
