Clinical Health Coach

The Clinical Health Coach will promote individual and community health by assisting individuals and communities to adopt healthy behaviors, collecting and analyzing data, and providing individual or group counseling and education.

Requirements

• Initiate contact with employees and/or spouses
• Assist with care navigation, connecting to programs and services, and follow through to ensure compliance
• Maintain communication with employees and/or spouses
• Provide individual or group counseling and education
• Identify barriers and develop treatment plans
• Engage and maintain relationships within a diverse population
• Act as a resource for resolving questions or concerns
• Monitor health education programs for effectiveness
• Maintain accurate documentation on patients
• Design, implement, and evaluate health education materials
