Social Worker Grade 2 - Admitted Subacute@Home

We are seeking a Social Worker Grade 2 to join our team at St George's Health Service of St Vincent's Hospital Melbourne. The successful candidate will be responsible for developing, implementing, and delivering a high-quality social work service to patients and families across GEM@Home and Rehab@Home. This is a challenging and rewarding work environment with a focus on wellbeing initiatives and a healthy work/life balance.

Requirements

• Bachelor of Social Work Degree/ Qualifying Masters Degree
• Eligible for membership of the Australian Association of Social Work (AASW)
• Current Victorian Drivers License
• Advanced social work skills as evidenced by four or more years post-qualifying professional experience in an inpatient hospital setting
• Demonstrated knowledge of the application of social work theory, practice frameworks and principles, in the context of the current health care environment
• Experience with NDIS applications and My Aged Care
