Lead case coordinator providing comprehensive mental health services to at-risk youth and families. Responsible for assessment, plan development, and implementation of client plans. Provides services in multiple settings and coordinates care with external providers.
Requirements
- Master's degree in Social Work, Psychology, or Counseling
- 1 year of experience providing services to youth and their families
- Registration as an Associate Social Worker (ASW), Associate Marriage and Family Therapist (AMFT), Associate Professional Clinical Counselor (APCC)
- Registration with CA-BBS as Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC) credential
