REHABILITATION CASE COORDINATOR I (UMP)

The Division of Rehabilitation Services is seeking a Rehabilitation Case Coordinator who will work with counselor(s) and the case management system to determine daily priority of customer service delivery. The Rehabilitation Case Coordinator will assist applicants, program participants, family members/guardians/care givers, vendors and referring agencies with understanding program rules, policies, eligibility criteria and other aspects of the Home Services Program.

Requirements

• High school degree
• Two years of clerical related experience
• Ability to travel for training
