REHABILITATION CASE COORDINATOR I (UMP)

The Division of Rehabilitation Services is seeking a Rehabilitation Case Coordinator who will work with counselor(s) and the case management system to determine daily priority of customer service delivery, and assist applicants, program participants, family members/guardians/care givers, vendors and referring agencies with understanding program rules, policies, eligibility criteria and other aspects of the Home Services Program.

Requirements

• Requires knowledge, skill and mental development equivalent to the completion of four (4) years of high school.
• Requires two (2) years of clerical related experience that demonstrates incorporation of working-level proficiency and accuracy in performing keyboarding functions.
• Requires ability to travel for training.
