The Clinical Pharmacist provides comprehensive pharmaceutical care to patients birth to death by optimizing medication therapy, providing disease state prevention and management, and promoting health and wellness.
Requirements
- Bachelor's Degree in Pharmacy from an accredited school of pharmacy.
- Successful completion of an ASHP accredited residency plus one year of hospital or relevant clinical experience, or completed equivalent clinical followship, or 3 years of relative work experience.
- Pharmacist licensure within 90 days of hire, Basic Life Support certification within 90 days of hire, Advanced Cardiac Life Support certification within 90 days of hire, vaccine certification within one year of hire, and ASHP certification within 2 years of hire or clinical certification from an accredited organization or board certificate within 2 years of hire.
