The role assists patients in accessing community services and supports medical providers and the management team with community outreach programs.

Requirements

• 2+ years combined of related education, experience, or certification
• Community Health Worker Certification required for certain states
• Efficient and reliable transportation, including an active driver's license
• Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms
