Job Description
The MAT Licensed Practical Nurse/Care Manager is responsible for providing comprehensive primary, preventative and behavioral health services to patients, including MAT intakes, patient care plans, crisis management, and case management.
Requirements
- High School Diploma/GED
- Valid New York State LPN License
- CPR Certification
- Infection Control Certification
- 1 year of experience (preferably 2-4 years)
- Driver's License
