Requirements

• Providing therapy to adults with I/DD who are involved in Medicaid Waiver services
• Conducting assessments and formulating individualized multidisciplinary treatment plans to guide services in homes and community settings
• Consulting with and training parents and host-home providers on therapeutic interventions and behavioral programs
• Working with treatment team members and case managers; on-call telephone support for crisis and emergency situations during normal work hours
