RN Case Manager Per Diem - (Temple University Hospital)

The RN Case Manager will collaborate with patients, physicians, and the interdisciplinary team to facilitate efficient and cost-effective care, monitor the level of care, and propose a transition plan within 48 hours of admission.

Requirements

• Bachelor's Degree in Nursing (BSN) or a combination of relevant education and experience
• 1 year experience in acute care Case Management setting or Utilization Management
• PA Registered Nurse License
