Pharmacist

Pharmacist position at Carondelet St. Josephs Hospital, responsible for medication selection, dosing, administration, and dispensing, as well as providing drug information and education to medical staff and patients.

Requirements

• Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.
• Compounds and dispenses medications for accurate delivery to patients.
• Provides drug information and education to medical staff and patients.
• Communicates with physicians and other clinicians as necessary to facilitate care.
• Accurately enters/verifies medication orders and performs final check prior to dispensing of all medications.
• Performs and documents clinical activities and interventions accurately.
• Supervises and directs pharmacy support personnel to provide timely, comprehensive, and cost effective pharmaceutical services.
• Participates in hospital safety/quality programs and assists in training new staff and students.
• Responds to code arrests and other emergent situations where pharmacist assistance is needed.
• Performs essential duties of pharmacy manager or director in his/her absence when the pharmacy clinical coordinator is not available.
