The Pharmacist provides pharmaceutical care services to patients, including oversight, monitoring, preparation, dispensing, and documentation of patient medication therapy. Additionally, provides drug information, patient information, and oversight of information and dispensing systems.
Requirements
- Review and monitor patient's medication therapy and orders written for safety and efficacy
- Advise patient care team on patient's medication therapy and document such interventions
- Order verification and performing medication profile reviews
- Prepare, label, and dispense medications for patients
- Supervise the preparation, maintenance, and dispensing process of drugs and supplies
- Maintain relationships with medical and nursing staff
- Provide education to patients on the use of medications
- Participate in the education and training of healthcare professionals
- Maintain professional competence with pharmacy department and state licensing requirements
