Advanced Practice Provider (Nurse Practitioner/Physician Assistant)

The Advanced Practice Provider (Nurse Practitioner/Physician Assistant) performs physical exams, creates medical treatment plans, and collaborates with patients and providers to create exceptional patient experiences. The Medical Director assists with the direction and delivery of medical care and oversees the development of clinically relevant education and process improvement.

Requirements

• Provide evidence-based, episodic care to patients
• Work within a defined scope of practice and under the authority of supervising physician(s) or by specific protocol(s)
• Order and interpret diagnostic tests as needed
• Partner with patients to determine, plan, and initiate appropriate treatment
• Safely prescribe and/or dispense medications
• Document medical information of patients and reviews patients' histories at each visit
• Evaluate response to treatments and medications
• Consults, collaborates, and refers with other health care providers when appropriate
• Counsels' patients and performs patient education
• Perform minor procedures
• Manage pediatric (3 months +) and adult patients for both acute and chronic conditions
• Actively maintain knowledge of current medical research and trends
• Provides administrative support or cross-coverage for reviewing laboratory tests, results, and prescription clarifications
