Job Description
The Care Services Coordinator is responsible for managing care services programs for individuals with ALS and their families within a designated territory area. The position involves facilitating education, information, benefit, and clinical resource navigation assistance, and providing referrals to ALS Association programs and healthcare providers.
Requirements
- Bachelor's Degree or related experience in social work, case management, health administration, allied and public health, nursing, or a related area
- Minimum of 3 years related professional experience including direct care and/or case management experience
- Knowledge of Medicare, Medicaid, patient assessment skills, care planning, and case management preferred
