Job Description
The Care Services Manager is responsible for developing professional and community relationships to provide care and support to those living with ALS. The role includes managing the care services program, facilitating education and resource navigation assistance, and collaborating with multidisciplinary clinics and community agencies.
Requirements
- Conduct specialized care consultations and needs assessments for people living with ALS and caregivers
- Manage a portfolio of patients and assure patient and caregiver needs are met
- Provide information on services and resources available to persons with ALS and their families
- Coordinate referrals to ALS Association programs and healthcare providers
- Facilitate resource support group meetings and engage participants
- Work with Managing Director Care Services/Care Services Director to evaluate care services programs
- Ensure reports are accurate and timely
- Recommend current or future programs to Managing Director Care Services/Care Services Director
