The Bradley School is a licensed private academy educating students with special education needs. We are seeking a Transition Coordinator who is motivated, compassionate and has a calling and passion for helping children.
Requirements
- Oversee the training of staff and ensure consistent, high-fidelity implementation of the transition curriculum
- Serve as the primary liaison between the school and external job placement sites, vocational programs and internship opportunities
- Conduct evaluations to assess program effectiveness, fidelity of implementation, and alignment with student transition goals
- Collaborate with county and district Office of Vocational Rehabilitation (OVR) liaisons to support individualized student transition planning and post-secondary outcomes
- Assists in developing individualized transition plans
