Physiotherapist (PT) - Community Based - East Region

The Care Company is seeking a skilled and dedicated Physiotherapist (PT) to join their growing team in a community-based model. This position requires in-person home visits with vulnerable community members.

Requirements

• Provide follow-up sessions focused on mobility, gait training, strength/balance retraining, pain management, and functional progression
• Develop, implement, and adjust individualized treatment plans in alignment with client goals
• Collaborate with OTs, PSWs, nurses, physicians, and HCCSS/OH@H care coordinators to support coordinated care
