Physiotherapist (PT) - Community Based - East Region

The Care Company is seeking a skilled and dedicated Physiotherapist (PT) with strong home and community care experience to join our growing team. This position is ideal for therapists who value flexibility, autonomy, and meaningful client relationships within a community-based model.

Requirements

• Provide follow-up sessions focused on mobility, gait training, strength/balance retraining, pain management, and functional progression
• Develop, implement, and adjust individualized treatment plans in alignment with client goals
• Collaborate with OTs, PSWs, nurses, physicians, and HCCSS/OH@H care coordinators to support coordinated care
