Transition Specialist

The Transition Specialist provides comprehensive services to individuals detained in city jails and supports their successful reentry into the community through the Pathway Home 6 program. This role includes assessment, planning, coordination, and delivery of evidence-based supports, crisis intervention, and collaboration with internal and external stakeholders to ensure continuity of care.

Requirements

• Bachelor's degree from an accredited institution preferred; minimum of associate's degree or equivalent credential required
• Minimum of 2 years of experience in career services, case management, workforce development, or social services OR in related fields
• Ability to pass security clearance with NYC Department of Corrections
