Job Description
The Case Manager in the Children's Outpatient Program (COP) is responsible to provide assessment, linking, coordinating and monitoring, family education and support, transportation, treatment planning and follow-up services to children and families.
Requirements
- Bachelor degree in social work, psychology, or related human service field from an accredited social services program within a 60-day training period beginning on the date of hire
- Children’s Mental Health Professional credential within 120 days of hire
- Valid chauffeur’s license and favorable driving record
