Job Description
Make a bigger difference by working as a Navigator/Case Manager in the MHOTRS clinic, providing integrated care to members of the community with behavioral health challenges and individuals with co-occurring substance use disorders.
Requirements
- Work collaboratively with clients and treatment providers
- Provide outreach to clients to facilitate engagement in services
- Complete documentation within required time frames and in required systems
