The Mental Health Center is seeking a Peer Support Specialist - Homeless Outreach to join their team. As a Peer Support Specialist, you will provide recovery-oriented support to individuals in the community living with Severe and Severe Persistent Mental Illnesses. You will work with a multi-disciplinary team to support patients in achieving treatment goals and objectives.
Requirements
- Working with a multi-disciplinary team
- Applying stages of change and motivational interviewing skills
- Using lived experience to engage individuals in identifying and working toward goals
- Providing support to individuals that fosters independence, autonomy, and self-advocacy
- Maintaining and completing all documentation
- Complying with regulatory confidentiality requirements
- Providing support that is sensitive to diverse cultures, languages, and recovery models
