Care Manager (RN)

The Clinical Case Manager facilitates an assigned group of patients through the health care system with the primary focus of efficient, effective, compassionate inpatient care and a safe transition to the outpatient setting.

Requirements

• RN license
• Bachelor of Science in Nursing
• 2 years clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations
• Prior case management experience
