Care Manager (RN) - University Hospital

The Clinical Case Manager monitors the patient care process and physiological needs; documents, communicates, and collects data to evaluate and improve timeliness and appropriateness of care.

Requirements

• RN license and Bachelor of Science in Nursing required
• 2 years clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations required
• Prior case management experience strongly preferred
