Job Description
The CBHH Care Coordinator is a direct service staff position for our Children’s Behavioral Health Home (CBHH). This position contributes to program outcomes through the provision of care coordination for CBHH members supporting a client-oriented, integrated health service that strives to improve health outcomes, cost effectiveness, and enhanced consumer engagement.
Requirements
- A Bachelor’s in Social Work preferred or a Bachelor's Degree in a related field
- At least one year of relevant experience working in human services with families and children
- Ability to work independently and collaboratively
- Ability to work flexible hours including evenings
- Computer proficiency with Microsoft Office and email
