The Wraparound Care Coordinator is a direct service staff position for the community services area, providing home-based and community-based case management services to youth and families with a mental health diagnosis or developmental disability.
Requirements
- Bachelor's Degree in Social Work or related field
- Knowledge of serious and persistent mental illness, substance abuse, and trauma
- At least two years' experience working with families and children, preferably within the mental health system
- Ability to work independently and collaboratively
- Ability to work flexible hours including evenings
- Computer proficiency with Microsoft Office and email
- Good driving record
