Care Coordinator - Wraparound Team

Care Coordinator for youth enrolled in the Wraparound with Intensive Services (WISe) program. Collaborate with families and multidisciplinary teams to plan and facilitate treatment, rehabilitation, and support services.

Requirements

• Bachelor's Degree in Psychology, social services, or behavioral health field
• Agency Affiliated Counselor Registration
• Valid driver's license and reliable transportation
• 2 years of job-related experience (preferred)
