Job Description
KMHS is seeking a YAT Team Care Coordinator to provide recovery-focused services for clients as part of a multidisciplinary treatment team. In this role, you will plan and deliver individualized care, support families, and respond to crisis needs while maintaining strong collaboration across systems of care.
Requirements
- Provide care coordination services for an assigned caseload, including individual, group, and family counseling, crisis intervention, and case management.
- Develop, implement, evaluate, and revise individualized treatment goals in collaboration with the treatment team.
- Educate and support families, advocate for client preferences, and consult with community agencies to support continuity of care.
