Job Description
Care Coordinator to join our Flexible Assertive Community Treatment (FACT) team, providing comprehensive case management, coordinating services, and supporting clients in achieving stability, recovery, and independence.
Requirements
- Provide active case management to address housing, health and hygiene, finances, food sufficiency, legal requirements, social skills, recreation, and activities of daily living.
- Participate in individualized treatment planning, ensuring plans reflect recovery-oriented goals and client input.
- Use recovery-focused language and clinical strategies to help clients strengthen independence and quality of life.
- Engage and motivate clients to make behavioral changes that support long-term recovery.
- Provide psychoeducation to clients and families regarding diagnoses, treatment, and recovery from psychotic disorders.
- Document progress and treatment activities in a timely, accurate manner that connects case management to treatment goals.
- Participate in daily staff meetings and scheduled treatment plan reviews.
- Support clients in reducing or abstaining from substance use, providing sober coping skills and linking to KMHS co-occurring treatment when needed.
- Apply situational awareness to evaluate safety and reduce risks in client homes and community settings.
- Collaborate with Mobile Crisis Outreach Team, law enforcement, and other partners during crisis intervention as directed by supervisor.
- Coach clients in independent living skills such as shopping, cooking, cleaning, budgeting, transportation, and socialization.
- Serve as primary clinician for up to 25 clients, managing treatment planning and documentation.
