Job Description
The registered nurse utilized the nursing process to coordinate the interdisciplinary approach to patient care, promoting and restoring patient's health by completing the nursing process; collaborating with physicians and team members; and providing physical and psychological support to patients and their families.
Requirements
- Consistently meets work schedule
- Demonstrates critical thinking utilizing all aspects of the nursing process
- Maintains a safe environment for patients
- Coordinates health care services with hospital, vendors, physician's offices and other institutions
- Answers patient/significant others phone calls, evaluate patient's needs, and triage patient care
- Provides patient/family education using resources available
- Acts as liaison between patient, significant others and health team members
- Identifies area for professional growth
- Participates fully as a member of the Patient Centered Medical Home Team
