Job Description
The LPN Care Coordinator provides patient care by serving as a liaison between patients and providers, coordinating treatment plans, and ensuring access to medical resources. This role focuses on improving patient outcomes through effective communication, patient education, and accurate record keeping. The Coordinator will coordinate transitions between healthcare facilities, perform routine updates on patient needs, and ensure patients have access to necessary care resources.
Requirements
- Must be a graduate of an accredited LPN program
- Must have a Pennsylvania Practical Nursing license
- Must have an active PA Practical Nursing License
- Experience in a health center setting is a plus
- BLS Certification
