Requirements
- Rounding and managing clinical care of patients at assigned SNF.
- Prescribing medications and treatments for residents in SNFs.
- Providing high-quality, patient-centered primary care services to patients in SNFs.
- Timely documentation of all patient care/encounters.
- Availability for phone/secure messaging consultations.
- Reliable transportation.
