Job Description
Theoria Medical is seeking a Part-time, in-person Nurse Practitioner to provide high-quality, patient-centered primary care services to residents in skilled nursing facilities (SNFs). Requirements
- Round and manage clinical care at assigned SNF.
- Prescribe medications and treatments for SNF residents.
- Provide timely documentation of patient care.
- Be available for phone/secure messaging consultations.
- Complete documentation in proprietary EHR system, ChartEasyTM.
