Requirements
- Foster trusting relationships with individuals served, emphasizing empathy, effective communication, and compassion.
- Develop, review, and monitor individualized care plans aligned with the unique needs and goals of each individual.
- Coordinate services and collaborate with interdisciplinary teams to ensure seamless delivery of care.
- Ensure compliance with lead agency guidelines and maintain meticulous, up-to-date documentation.
- Provide support, education, and guidance to individuals and their families, enabling informed decisions regarding their care and increasing their ability to advocate for their needs.
