Transitions Care is a leading hospice organization that provides 24/7 access to services in the comfort of the patient's home. As an Advanced Nurse Practitioner, you will be responsible for the direction and oversight of the care and treatment of hospice patients.
Requirements
- Master’s Degree in Nursing
- Current license to practice in the state of employment and Controlled Substance if required by the state of employment
- Active DEA License with no restrictions
- One (1) + year of home care or hospice experience
