Job Description
Transitions Hospice provides compassionate and unhurried hospice care, focusing on the whole person and protecting dignity, comfort, and moments that matter most. As an RN Case Manager, you'll be the clinical anchor for patients and their families, coordinating care, guiding the interdisciplinary team, and showing up when it matters most.
Requirements
- Active RN license in the state of employment
- Graduate of an accredited school of Nursing
- One (1) + year Hospice, home health or related organization experience preferred
- Compassion for patients and families navigating end-of-life care
- Strong clinical judgment and the ability to work independently in the field
- Excellent communication skills across care teams and families
